Evaluation and CE Summary

Thank you for participating in the 5th Annual EMS CE Fall Forum. To improve future conferences and other educational events, where we may continue to use online technology as well as gathering in-person, we are asking for your feedback.

To receive your CE Certificate or Certificate of Attendance, both the Evaluation Form and Contact Hour (CE) Summary Form MUST be completed

Please note your name and email are optional on the Evaluation Form.

The Contact Hour (CE) Summary Form MUST be thoroughly completed and submitted by December 15, 2024 to avoid a late fee charge.

You MUST watch the Video Below for further instruction and to gain PASSWORD.

Once you have the PASSWORD, enter it in the Box Below the Video. You will then access Evaluation form which must be completed for CEs. Upon completion of the Evaluation form, you will be redirected to the Contact (CE) Summary Form which also MUST be completed.

NOTE: After you complete the Evaluation form, you MUST CLICK SUBMIT in order for it to be recorded and to be automatically redirected to CE Summary Form. Same for the CE Summary form, you MUST CLICK SUBMIT to record it.
